Labral Pull-Through Technique for Arthroscopic Labral Reconstruction of the Hip

Background: The goal of hip labral reconstruction is to restore the suction seal, requiring graft be precisely secured along acetabular rim. When performed arthroscopically, controlling as it suspended in joint between 2 cannulas can challenging, making fixation optimal position seal difficult achieve. Indications: Labral reconstruction/augmentation indicated for patients with a deficient/dysfunctional labrum when repair alone cannot restore/maintain seal. Technique Description: Following diagnostic arthroscopy, debridement, and measurement deficiency (30 mm), 70 mm tibialis anterior allograft was tubularized whipstitch prepared long suture tails at both ends single mattress stitch placed through medial end. Holes anchors were pre-drilled. Viewing from anterolateral portal, introduced down sled modified pulled joint, guided into posterolateral portal cannula via instrumentation distal accessory portal. With initial achieved medially stitch, directing while achieving coverage deficient area facilitated by excess length cannula. Results: not sufficient. Comparative studies matched cohorts have demonstrated comparable survivorship patient-reported outcomes groups, similar rates patient acceptable symptomatic state minimal clinically important difference return sport high-level athletes.
